Appointments
If you would like to make an appointment, please contact our office by phone at 888-7561or email. Our office hours are monday through thursday 8:30 am to 5:00 pm . Friday and early hour appointments are available so please call for details. Our facilitates galdly accepts emergency calls and appointments, especially for new patients seeking help. Please plan on showing up a few minutes early to fill out any necessary forms.
Financial
We will be more than happy to submit all insurance forms for you and help you recover the most from your benefits.We accept most major insurance plans. We will do everything we can to help you afford the treatment you need and want. For patients who want or require extensive work, a complete payment plan is designed with an appropriate payment schedule. Forms of payment accepted by the office are check, cash, major credit card and financing is available.
Cancellation
We know how vaulable your time is. So we ask If you are unable to keep an appointment, please provide us with at least 48 hours notice. We ask for this advance notice so that we can offer this appointment to another patient. A fee will be charged if a patient does not show up for an appointment without sufficient notice.
